Summary
The Home Health Aide enables patients to remain safely and comfortably in their homes by monitoring their condition, providing personal care, assisting with daily living needs, and supporting families with education and guidance.
Responsibilities
Patient Care & Monitoring
Observe and record physical and mental condition, including intake, output, and activity.
Notify nursing supervisor of changes or unusual conditions.
Assist with personal care needs such as bathing, dressing, and grooming.
Support mobility and independence by teaching use of canes, walkers, and adaptive equipment.
Daily Living Support
Prepare and serve meals and snacks according to dietary requirements.
Provide light housekeeping and laundry services.
Run errands and shop for food or household items.
Family Support & Education
Teach family members safe ways to lift, turn, and reposition the patient.
Provide guidance on nutrition, hygiene, and maintaining a safe home environment.
Safety & Compliance
Maintain a clean, secure, and healthy environment by following safety and infection control standards.
Adhere to agency policies, procedures, and federal/state/local regulations.
Documentation & Communication
Accurately document care and observations in patient records.
Communicate effectively with supervisors and the care team.
