Job Summary
We are seeking compassionate and dedicated Community Support Team Qualified Professional to join our team in providing exceptional care to individuals in various settings, including home environments, group homes and assisted living facilities. The ideal candidate will be responsible for leading a team whose function is to provide direct support to adults with a Diagnostic and Statistical Manual of Mental Disorders Fifth Edition (DSM-5) diagnosis of mental illness, substance use, co-morbid disorder and who have complex and extensive treatment needs
Responsibilities
- Driving the delivery of this service
- Providing individual therapy for beneficiaries served by the team
- Behavioral interventions such as modeling, behavior modification, behavior reversal
- Designating the appropriate team staff so that specialized clinical expertise is applied as clinically indicated for each beneficiary
- Providing and coordinating the assessment and reassessment of the beneficiary’s clinical needs
- Providing clinical expertise and guidance to the CST members in the team’s interventions with the beneficiary
- Providing the clinical supervision of all members of the team for the provision of this service. (An individual supervision plan is required for all CST unlicensed members .)
- Determining team caseload by the level of acuity and the needs of the beneficiary served
- Facilitating weekly team meetings of the CST
- Monitoring and evaluating the services, interventions and activities provided by the team
- Completes functional needs assessment(s) to determine the scope and anticipated outcomes to the services
Experience
Preferred candidates will have experience in the following areas:
- A Licensed Professional (LCSW, LCAS, or LCMHC) who has the knowledge, skills and abilities required by the population and age to be served.
- An Associate Level Professional actively seeking licensure may serve conditional upon being fully licensed within thirty (30) calendar months from the effective date of hire.
- The team leader shall meet the requirements specified for licensed clinician, according to 10A NCAC 27G.0104 (12).
- A minimum of one (1) year of documented experience with this population
- Must be able to provide direct clinical interventions with each client; use the team approach for structured, face-to-face, scheduled therapeutic interventions to provide support and guidance across multiple functional domains including emotional, medical, and health.
- Ability to provide services as defined by service definitions, County Program/Area Authority/Local Management Entity guidelines and NC Division of MH/DD/SAS rules and regulations
- Must successfully complete the minimum training specific to the required components of the Community Support Team service definition within the required days of hire
- Must have the skill, knowledge and experience to provide planned interventions, a variety of planned clinical rehabilitative interventions, support ongoing treatment and functional gains, symptom monitoring and self- management of symptoms
- Must be on call and accessible during periods of crisis for clients receiving services and available for face-to-face contact within two (2) hours of emergency/crisis notification
- Deliver services directly to client in their living environments as directed by the client’s Person-Centered Plan
- Must have positive attitude and view the individual as first priority.

- Valid driver's license and ability to obtain and maintain appropriate car insurance coverage.
- First aid and CPR certification
