What are the responsibilities and job description for the DD Waiver Case Manager position at Visions Case Management?
The Developmental Disabilities Waiver provides home and community based services to individuals with I/DD. The case manager coordinates these services and is the liaison between the individual, state of New Mexico, and various service providers. Duties include monthly face to face visits with clients, drafting and submitting annual plans and budgets, facilitating team meetings, maintaining current client files, and adhering to timelines and standards.
Internally Case Managers are required to attend a staff meeting that could be in Santa Fe or Albuquerque once a month, along with one on one supervision, support annual reviews (audits), and to follow the agency’s Quality Assurance Plan and Policies and Procedures.
Ideal candidate is detail oriented, flexible, responsive, computer savvy and highly self-motivated. Pay depending on caseload.
Candidate must be willing to travel throughout the northwest region of New Mexico for client visits and have reliable transportation.
Degree requirements: Bachelor’s or Master’s degree in social work, psychology, counseling, nursing, special education, or closely related field, plus one year experience working with individuals with intellectual/developmental disabilities. If the degree is in another field, 6 years of experience with individuals with intellectual/developmental disabilities are required.
We are a subcontractor model, so there are no traditional salary or benefits. We are paid a flat rate from the state of new mexico of $314 per person in services. We start subcontractors at 70% of that flat rate per person on their caseload. Here are some examples of caseload pay: 10 person caseload $2,198, 20 person caseload $4,396, 30 person caseload $6,594. Subcontractors get a 1% rate increase per year up to 85%. Visions pays NM Gross Receipts tax, and subcontractors are responsible for state and federal income taxes. Several expenses can be tax deductible.
