Key Responsibilities
- Conduct comprehensive assessments of patients health status and assist in developing individualized care plans.
- Administer medications, perform skilled nursing treatments, and provide direct patient care in clients homes.
- Monitor and evaluate patients conditions, promptly identifying and reporting any changes in health status.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to coordinate care.
- Educate patients and family members on disease prevention, medication administration, and overall health management.
- Provide emotional support and foster a caring, positive environment for patients and families.
- Accurately document all care provided in accordance with company policies, regulatory standards, and EMR requirements.
- Maintain compliance with all applicable federal, state, and professional nursing regulations and standards of care.
- Current, active LPN license in the State of Virginia.
- Minimum of 2 years of experience in home health or a related clinical setting.
- Strong clinical assessment, documentation, and critical-thinking skills.
- Excellent communication, organizational, and interpersonal abilities.
- Proficiency with Electronic Medical Records (EMR) systems.
- BLS certification (current).
- Reliable transportation and a valid drivers license.
- Must pass background checks and drug screening.
