Job Overview
We are seeking a dedicated and compassionate Social Worker to join our team. The ideal candidate will have a strong background in social work, with experience in various settings including hospitals, outpatient clinics, and community care. This role involves providing support and counseling to individuals and families facing challenges related to mental health, addiction, and other life circumstances. The Social Worker will be responsible for conducting assessments, developing treatment plans, and facilitating both individual and group therapy sessions.
Duties
- Conduct comprehensive patient assessments to determine needs and develop appropriate treatment plans.
- Provide individual and group counseling sessions focusing on behavioral health, mental health counseling, and crisis intervention.
- Collaborate with healthcare professionals to ensure coordinated patient care across various settings including inpatient, outpatient, assisted living, and hospice care.
- Maintain accurate medical documentation and records in compliance with HIPAA regulations.
- Engage in clinical research to enhance treatment methodologies and contribute to the field of social work.
- Utilize EMR systems for data collection and management of patient information.
- Implement therapeutic techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and applied behavior analysis (ABA).
- Support patients dealing with developmental disabilities, PTSD, Alzheimer’s care, dementia care, and other specific needs.
- Participate in case management activities to monitor patient progress and adjust treatment plans as necessary.
- Advocate for patients’ needs within the healthcare system while providing education on available resources.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution is required.
- Valid state licensure or certification in social work practice.
- Experience in child & family counseling, addiction counseling, or behavioral therapy is preferred.
- Strong knowledge of medical terminology and experience with medical records management.
- Familiarity with ICD-9/ICD-10 coding systems is a plus.
- Proven ability to conduct diagnostic evaluations and provide crisis management support.
- Experience working with diverse populations including geriatrics, pediatrics, individuals with disabilities, or special education needs is advantageous.
- Excellent communication skills with the ability to build rapport with clients from various backgrounds.
