- Monitor clients’ health status and report any changes or concerns to healthcare professionals or family members promptly.
- Support clients with meal preparation and nutritional needs tailored to their dietary requirements.
- Help clients stay engaged through recreational activities, hobbies, or outings as appropriate.
- Follow care plans meticulously, ensuring all services are delivered safely and respectfully.
Experience
- Prior experience working as an in-home caregiver or in a similar personal care role is preferred.
- Experience supporting clients with geriatrics or post-acute care needs is highly valued.
- Knowledge of medical-surgical procedures, outpatient and clinic settings enhances your ability to deliver comprehensive care.
- Inpatient and sub-acute care experience supports delivering consistent, high-quality support in various settings.
- Certification or training related to geriatrics, post-acute care, or outpatient services is advantageous for providing specialized assistance.
