Role Summary
As a Community Health Coordinator, you will support Care Management by coordinating Community Health Worker (CHW) visit schedules and producing accurate schedules, reports, and documentation for Supervisor use.
Key responsibilities include scheduling home/field appointments, calling members to arrange CHW visits with a Service Excellence mindset, maintaining schedules in Outlook and Excel, entering staff/client data in databases, tracking statistics and reports, and completing general office support (documents, filing, shredding, mail, reception/phones). You will also identify improvement opportunities and escalate issues as needed.
Required qualifications/skills:
- High School diploma required; Associate's preferred
- Valid NYS ID or NYS Driver License
- 2-3 years healthcare experience preferred
- Bilingual preferred (Spanish, Russian, Chinese, Bengali, Creole)
- Proficiency in Microsoft Word, Outlook, and Excel
- Strong written and verbal communication skills

You'll also handle routine administrative needs as they arise, respond professionally to incoming calls, and communicate clearly when something needs attention or escalation. The pace is steady and detail-driven, with work-from-home flexibility while supporting members across New York.
