Patient Care Coordinator Duties and Responsibilities:
- Greets and welcomes new residents upon admission and assists with orientation to the facility, including touring the resident through their new home and reviewing available services and amenities.
- Assists residents with daily non-clinical needs to promote comfort, safety, and overall well-being.
- Supports residents with daily activities and routines in accordance with individualized care plans and personal preferences.
- Collaborates with Nursing and the Interdisciplinary Team to ensure new residents are introduced to their care team and that immediate needs are communicated and addressed.
- Assists with room readiness to ensure the resident’s environment is clean, organized, personalized, and welcoming upon arrival.
- Communicates resident concerns, preferences, and feedback to the appropriate department to ensure timely follow-up and resolution.
- Maintains positive rapport with residents and families, promoting a supportive and person-centered environment.
- Assists with admission-related processes and documentation as directed to ensure a smooth transition into the facility.
- Participates in daily team meetings or rounds as requested to provide updates on new admissions and resident needs.
- Supports resident engagement and encourages participation in facility programs and activities as appropriate.
- Perform all other duties, as assigned.
- Ability to relate positively, effectively, and appropriately and independently with patient and families.
- Experience working on Care Transition Unit would be preferred.
- Strong verbal and written communication skills in English to perform the tasks required.
- Sufficient computer skills required to perform duties.
- High school diploma or equivalent.
- At least one year of prior experience in a relevant role, preferred.
- At least one year of experience working in a healthcare setting.
- Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
- Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
- Meet all health requirements as needed.
