You will be the person who makes it manageable.
As a GUIDE Dementia Care Navigator, you’ll:
• Develop individualized, person-centered care plans
• Serve as the primary point of contact for patients and caregivers
• Provide regular check-ins (in-person and by phone)
• Connect families with community resources, transportation, meal programs, and support groups
• Offer education on dementia progression and behavioral management
• Help coordinate respite services to reduce caregiver burnout
• Navigate complex healthcare systems and coordinate across providers
• Ensure timely and compliant documentation in alignment with CMS GUIDE requirements
This is hands-on, relationship-driven work. You won’t be sitting behind a desk all day — you’ll be in facilities, on the phone, and in the community, building trust and making real impact.

What We’re Looking For
• Bachelor’s degree in Social Work or related field
• Strong empathy and communication skills
• Comfort educating families and discussing difficult topics
• Knowledge of dementia progression and behavioral symptoms
• Highly organized and proactive
• Comfortable coordinating across multiple providers and systems
• Valid driver’s license and ability to travel locally to senior living communities
Bonus Points For
• Experience in case management or geriatric care
• Familiarity with Medicare or CMS models
• Prior dementia-focused experience
