- Knowledge of social service policies, admissions, and PASSAR screens
- Identify psychosocial needs during admission and ongoing
- Complete MDS sections and develop personalized care plans
- Advocate for Resident Rights and educate staff on abuse prevention
- Participate in discharge planning and documentation
- Coordinate services with outside agencies
- Utilize electronic health records effectively
- Engage in QAPI processes and maintain professional competence
- PA license, Bachelor’s in Social Work or Human Services, with 1 year supervised healthcare experience
