Registered Nurse (In-Home Care)
Provide compassionate, patient-centered clinical care to elderly and chronically ill clients in their homes. Conduct thorough assessments, develop and update individualized care plans, and administer medications and prescribed treatments.
Coordinate care with physicians, caregivers, and interdisciplinary teams to support safe, effective outcomes and continuity of services.
Ensure strong adherence to regulatory requirements and agency policies, and maintain accurate, detailed documentation of clinical observations, care provided, and plan updates.
