Responsibilities
- Coordinate care within an interdisciplinary team to support independence and quality of life for elderly patients.
- Deliver strong direct patient care and care management in a community setting.
- Ensure compliance with BLS, First Aid, and other certification requirements.
- Maintain documentation and support patient well-being through personalized care plans.
- Collaborate with team members to optimize patient outcomes.
- BSN degree, RN license, and BLS certification.
- At least 1 year of experience working with the frail or elderly; 3 years of nursing management in community-based care preferred.
- Basic Cardiac Life Support certification within 30 days of start.
- Strong communication, care coordination, and geriatric care skills.
- Commitment to compassionate, patient-centered care.
