- Manage acute and chronic conditions, develop care plans, coordinate referrals, and provide clinical consultation.
- Lead quality improvement efforts, develop workflows, and support team effectiveness.
- Participate in documentation, follow-up, patient education, and social needs assessment.
- Mentor staff, attend meetings, and contribute to clinic procedures and quality activities.
