Role: Home Health Physical Therapist

Responsibilities

  • Provide one-on-one rehabilitative care in patients' homes
  • Evaluate mobility and functional status
  • Develop individualized treatment plans
  • Deliver evidence-based therapy
  • Educate patients and caregivers on exercises and safety
  • Collaborate with an interdisciplinary team
  • Complete accurate electronic medical record (EMR) documentation
  • Support a patient-centric culture of empathy, respect, and integrity
  • Help patients regain independence at home
