Join our healthcare team as a Home Health Nurse Practitioner providing compassionate, personalized care directly in patients' homes.

  • Conduct comprehensive assessments, develop care plans, and perform treatments such as IV infusions and wound suturing.
  • Perform clinical procedures including catheterization, phlebotomy, and basic life support.
  • Educate patients on disease management, medications, and lifestyle modifications.
  • Utilize EHR systems like Epic or eClinicalWorks for documentation.
  • Coordinate with multidisciplinary teams for seamless care and discharge planning.

Requirements: Valid Nurse Practitioner license, experience in critical care preferred, proficiency with EMR/EHR systems, and ability to perform complex procedures. Experience with diverse populations and telehealth is advantageous.

This part-time, hybrid role in Salem, OR offers pay between $32.06-$38.61/hour and paid time off, focusing on delivering high-quality home-based healthcare.
