Home Health nurse practitioner

Join our healthcare team as a part-time Home Health Nurse Practitioner, delivering compassionate care directly in patients' homes.

• Conduct assessments, develop care plans, and perform treatments such as IV infusions and wound management.
• Perform clinical procedures, provide patient education, and utilize EHR systems like Epic or eClinicalWorks.
• Coordinate with multidisciplinary teams for seamless care and discharge planning.
• Requirements include a valid Nurse Practitioner license, experience in critical care preferred, and proficiency with telehealth and EMR systems.
