Position Overview

As a Case Manager with Veteran Pathways of New England, you will work closely with Veterans and service members to develop individualized stabilization plans that address housing, employment, and wellness needs. This role serves Veterans and their families across Cape Cod and Suffolk County, Massachusetts (MA), with a primary focus on outreach and client engagement in the Cape Cod area.

The position begins with an initial training period of approximately two months. Training may be completed in-person, remotely, or through a combination of both depending on operational needs.

This role combines direct client engagement with a significant documentation component. Case Managers are responsible for maintaining a full caseload while completing timely, accurate, and compliance-driven documentation for each client served.
