What You'll Do
As a Case Manager, you'll work directly with youth and families in their homes, schools, and communities to help them build stability, strengthen relationships, and achieve their goals.
Your responsibilities will include:
- Develop meaningful relationships with youth and families through a strength-based, trauma-responsive approach
- Manage an assigned caseload while partnering with families, schools, funding agencies, and community providers
- Provide mentoring, coaching, advocacy, and supportive services in homes, schools, and community settings
- Connect families with essential resources including housing, food, utilities, healthcare, and other community supports
- Help youth build life skills, improve social and emotional functioning, and participate in healthy recreational and educational activities
- Coordinate services with treatment teams and community partners to ensure continuity of care
- Develop treatment plans, document progress, and maintain accurate electronic records
- Participate in supervision, professional development, and team meetings
- Travel throughout the Cape Cod service area to meet families where they are
- Promote the ARC (Attachment, Regulation, and Competency) framework in every interaction with youth and families

Qualifications
- Bachelor's Degree
- Experience working with children, adolescents, and families
- Experience in human services, behavioral health, education, or social services
- Bilingual candidates are strongly encouraged to apply
