Vesa Health & Technology is seeking a Registered Nurse to support the Primary Care Clinic as a complex care coordinator at the Coast Guard Academy Medical Clinic in Cape May, NJ
**The clinic requires that the RN must have at least 1 year of primary care/family practice/public health outpatient experience in order to be considered.**
The program seeks to implement a patient and family centered health care delivery for primary care that is team-based, comprehensive, and designed to fully meet the complete primary care health and wellness needs of patients.
The RN complex care coordinator will manage complex care issues with high risk patients. The RN will track care, coordinate with providers and specialists, and enlist resources needed to serve the patient's needs. Other functions include assisting with training/education and vaccine clinic floating when needed.

Hours: Day Shift, Monday through Friday. The clinic is closed on weekends and the day of observance of Federal holidays.
Qualifications.
- Be a graduate from an accredited nursing program
- Possess a valid RN license in 1 of the 50 states.
- Possess a minimum experience of 3 years as a Registered Nurse within the last 5 years.
- 1 year experience working in an ambulatory based outpatient, primary/family practice environment.
