We are seeking a compassionate and reliable Registered Nurse (RN) to perform periodic check-ups on patients receiving care in their homes. In this role, you will ensure the ongoing health and well-being of home care clients by conducting routine health assessments, monitoring chronic conditions, educating patients and caregivers, and working closely with the care team to deliver high-quality, person-centered care.
Responsibilities:
Conduct routine wellness check-ups and health assessments at patients' homes
Monitor and evaluate patient condition and vital signs
Review and update care plans based on clinical findings
Provide education and support to patients and families on managing health conditions
Collaborate with physicians, home health aides, and care coordinators
Maintain accurate documentation and timely reporting of visits
Identify and report any changes in patient condition that require medical attention
Ensure compliance with healthcare regulations and safety standards
Qualifications:
Current and valid Registered Nurse (RN) license in NJ
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills
Reliable transportation and valid driver’s license
Ability to work independently and manage time effectively
Familiarity with electronic medical records (EMR) systems preferred
