What are the responsibilities and job description for the OCCUPATIONAL THERAPIST HOME HEALTH VISITS position at Vertus HealthCare?
We are seeking an Occupational Therapist to join our team to ensure that we continue to provide all patients with high quality, efficient care.
The Occupational Therapist provides high-quality, patient-centered care by managing the complete process of occupational therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team.
The Occupational Therapist completes patient assessments, and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience. The Occupational Therapist provides patient’s physical therapy to support patients throughout home health and the continuum of care.
Qualifications:
Education: Degree from an American Physical Therapy Association (APTA) accredited program required.
Experience: Minimum one (1) year of experience in health care setting performing similar work activities preferred.
Certification/Licensure: Licensed as a Physical Therapist by the state of Florida Allied Health Professionals required. BLS, CPR required.
