Location: Chesapeake, VA / Norfolk, VA (Onsite across 3-8 dialysis centers)

Schedule: Full-time, Monday-Friday | Daytime hours (8 AM - 5 PM)

Work Type: Onsite (no remote option)

As the founding NP in this market, you'll shape a first-of-its-kind initiative that integrates primary care, nephrology, and chronic disease management—bringing care to patients where they already receive dialysis. This is a clinical leadership role with significant autonomy and opportunities for innovation and future advancement.

Key Responsibilities

• Deliver comprehensive primary care to patients within assigned DaVita dialysis centers (10-12 patient visits per day).
• Manage chronic health conditions including diabetes, hypertension, COPD, heart failure, and other ESRD-related complications.
• Prescribe and titrate medications including insulin, antihypertensives, and statins according to evidence-based guidelines.
• Conduct annual wellness and preventive health evaluations.
• Coordinate with nephrologists, dialysis care teams, integrated care leaders, and local PCPs to optimize care.
• Develop clinical processes and workflows for a new integrated care model.
• Participate in innovation feedback and collaborate directly with DaVita leadership on program design.
  
  

Qualifications

Must-Have Requirements:

• Active Nurse Practitioner (NP) License in Virginia with autonomous practice authority.
• Minimum 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.
• Strong background in managing chronic conditions such as diabetes, hypertension, COPD, CHF, and kidney disease.
• DEA license with proven prescribing experience for chronic disease management.
• Local to Chesapeake/Norfolk, VA area (no relocation assistance).
  
  

Preferred Qualifications:

• Prior experience in dialysis, nephrology, or ESRD population management.
• Experience launching or developing new clinical programs or service lines.
• Comfortable working in a performance-based, results-driven model with clinical outcome incentives.
