Location: Chesapeake, VA / Norfolk, VA (Onsite across 3-8 dialysis centers)

Key Responsibilities
- Deliver comprehensive primary care to patients within assigned DaVita dialysis centers (10-12 patient visits per day).
- Manage chronic health conditions including diabetes, hypertension, COPD, heart failure, and other ESRD-related complications.
- Prescribe and titrate medications including insulin, antihypertensives, and statins according to evidence-based guidelines.
- Conduct annual wellness and preventive health evaluations.
- Coordinate with nephrologists, dialysis care teams, integrated care leaders, and local PCPs to optimize care.
- Develop clinical processes and workflows for a new integrated care model.
- Participate in innovation feedback and collaborate directly with DaVita leadership on program design.
Must-Have Requirements:
- Active Nurse Practitioner (NP) License in Virginia with autonomous practice authority.
- Minimum 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.
- Strong background in managing chronic conditions such as diabetes, hypertension, COPD, CHF, and kidney disease.
- DEA license with proven prescribing experience for chronic disease management.
- Local to Chesapeake/Norfolk, VA area (no relocation assistance).
- Prior experience in dialysis, nephrology, or ESRD population management.
- Experience launching or developing new clinical programs or service lines.
- Comfortable working in a performance-based, results-driven model with clinical outcome incentives.
