This role supports a participant-centered care model, focusing on improving independence, safety, and quality of life for elderly and frail populations through coordinated interdisciplinary care.
Key Responsibilities
Clinical Care
- Administer occupational therapy treatment programs as directed by OT
- Assist with vital signs, range of motion, and strength assessments
- Deliver therapeutic interventions to improve ADLs
- Work with elderly/frail participants with complex medical needs
- Promote independence through therapeutic activities
- Support cognitive, balance, endurance, and motor function development
- Collaborate with interdisciplinary team (IDT)
- Coordinate with therapists and healthcare providers
- Support care plan implementation
- Document therapy interventions and patient progress
- Assist with scheduling treatments and clinical visits
- Ensure compliance with care plans and documentation standards
- Associate's Degree - Occupational Therapy Assistant (Required)
- Active California OTA License or eligibility (Required)
- Minimum 2 years of OTA experience
- Experience working with elderly / frail populations
- Strong understanding of ADL-focused rehabilitation
- Experience in outpatient, acute, inpatient, or skilled nursing settings
- Experience in interdisciplinary care models
