Key Responsibilities
- Assess, plan, and manage care for at-risk patients in acute care settings.
- Coordinate multidisciplinary care and optimize resource utilization.
- Conduct discharge planning and follow-up to ensure continuity of care.
- Document patient interventions accurately and maintain regulatory compliance.
- Serve as a patient advocate and provide education to patients and families.
- Collaborate with physicians, nursing staff, and other healthcare professionals.
- Promote a culture of patient-focused, whole-person care.
- Participate in professional development and care management programs (e.g., TIPS or Case Management Orientation).
- Associate's Degree in Nursing (ADN) or Nursing diploma upon hire.
- Washington Registered Nurse (RN) License.
- Minimum 2 years of acute care experience in a Medical-Surgical or Inpatient setting.
- Bachelor's Degree in Nursing or higher (to be obtained within 3 years of hire).
- National certification in care management or area of specialty.
- 1 year of care management or utilization review experience in any setting or completion of TIPS / Case Management Orientation Program.
