- Build trusted relationships and help patients achieve their health goals.
- Partner with patients and the care team, including a Whole Health Coach, to develop personalized care plans.
- Provide preventive care, acute care, chronic disease management, and care coordination.
- Participate in team meetings and quality improvement initiatives that drive exceptional patient outcomes.
- Current, unrestricted NP or PA license (or ability to obtain prior to start)
- Board Certification as an NP or PA
- Current DEA registration
- Primary care, family medicine, or internal medicine experience preferred
- Passion for relationship-based, whole-person care
