Position Title: Community Resource Coordinator (CRC)

Reports To: VTH Leadership/Site Supervisor
Salary Range: $26.00 – $29.00 per hour (DOE)
Employment Type: (30 hours) Part-Time, Non-Exempt

VTH is committed to providing exceptional services to some of the most vulnerable populations within LA County through compassion, creativity, and authenticity. We prioritize referrals and services for individuals: with multiple contacts with the criminal legal system, are at high risk for recidivism, typically excluded or underserved by existing programs (including people who are transgender, people experiencing homelessness, immigrants, people living with HIV), and those disproportionally impacted by racial disparities in policing, arrests, and sentencing.

About the Role:

The Community Resource Coordinator (CRC) connects individuals and families with local programs, public benefits, and essential support services, including housing, food assistance, healthcare, financial assistance, job training, and harm-reduction resources. This role assesses community members’ needs, provides personalized service navigation, develops relationships with community partners, and helps ensure clients can access the resources available to them.

The Community Resource Coordinator may work in nonprofit organizations, government agencies, healthcare organizations, health plans, community centers, or other community-based settings.

Duties & Responsibilities include, but are not limited to:

Needs Assessment: Conduct one-on-one conversations with individuals and families to understand their needs, identify barriers to accessing services, and determine appropriate resources and support.

Service Navigation: Connect clients with community resources such as food pantries, housing assistance, healthcare services, public benefits, transportation, financial assistance, employment programs, and other supportive services.

Harm Reduction & Health Education: Provide education and information on safer-use practices, overdose prevention, sexual health, and other strategies that reduce health risks and support individual and community well-being.

Supply Distribution: Distribute harm-reduction and health supplies, including safer-use supplies, sexual health supplies, and overdose prevention supplies, in accordance with program policies and applicable regulations.

Overdose Prevention Education: Provide individuals, families, and community members with education on overdose prevention, recognizing signs of overdose, responding to an overdose, and accessing appropriate emergency and community resources.

Partnership Building: Develop and maintain relationships with local agencies, nonprofits, healthcare providers, service providers, local business owners, employment agencies, community organizations, and community leaders to strengthen referral networks and identify gaps in available services.

Resource Coordination: Help clients understand eligibility requirements, application processes, and available services, and provide follow-up to support successful connections to resources.

Community Outreach: Participate in outreach activities to increase awareness of available resources, harm-reduction services, and health education opportunities, particularly among underserved or hard-to-reach populations.

Data Tracking and Reporting: Maintain accurate records of client referrals, services provided, supplies distributed, educational activities, outcomes, and follow-up activities. Prepare reports and track program metrics to evaluate effectiveness and identify areas for improvement.

Event Planning: Organize and support community events such as health fairs, educational workshops, resource fairs, overdose prevention trainings, and resource distribution drives.

Resource Development: Help maintain current resource directories, referral materials, community resource guides, and educational materials.

Qualifications/Experience/Skill Set:

  • Harm Reduction, substance use, and engagement with PWUD, PEH or lived experience demonstrating sufficient training, education, and/or experience
  • Experience working with individuals, families, or communities in a social service, nonprofit, healthcare, government, public health, or community-based setting
  • Ability to assess needs and connect individuals with appropriate resources
  • Knowledge of or willingness to learn about harm reduction, overdose prevention, sexual health, and community health resources
  • Strong organizational skills and attention to detail
  • Familiarity with local community resources and public benefit programs and ability to build and maintain effective relationships with community partners
  • Ability to work with people from diverse backgrounds and with varying needs
  • Experience with community outreach, health education, or event coordination
  • Effective communication, active listening, interpersonal skills, and ability to work with high needs participants in a compassionate way without taking words, comments, or the actions of others personal
  • Ability to establish rapport and collaborate with participants to assist them with their own recovery goals and needs
  • Working knowledge of HIPPA, participant/client confidentiality and privacy (or willingness to complete HIPPA training within first 30 days from date of hire)
  • Willingness to continue learning and developing skills related to participant engagement, community outreach and engagement, and fundamentals of mental health/substance use screenings and referral processes
  • Bilingual (Spanish) preferred

Work Environment & Physical Requirements

· Regular use of computer, phone, and other standard office equipment.

· Must be able to lift 25 pounds occasionally, ability to work multiple hours on feet and sit comfortably to preform basic office/documentation work.

  • Ability to get in and out of mobile outreach vehicle independently and walk long distances as needed to engage with unhoused participants
  • Fast-paced drop-in center environment, with potential exposure to behavioral health crises, medical emergencies, and overdose response situations.
  • Requires compassion, flexibility, authenticity, initiative, crisis-response capacity, and a deep commitment to participant autonomy and dignity.

This position may involve working in an office, community center, healthcare setting, outreach environment, or other community-based locations. The role may include local travel, community outreach, supply distribution, educational activities, and occasional evening or weekend events depending on program needs.

Position Classification and Compensation

· FLSA Status: Non-Exempt (Hourly)

· Work Schedule: 40 hours per week, Monday–Friday (with flexibility for program needs).

· Overtime: Eligible for overtime pay in accordance with the Fair Labor Standards Act (FLSA) and California Labor Code.

Timesheets: Required for all work hours, travel time, and training attendance.
