What You’ll Do:
- Deliver top-notch care to patients in their homes, following their individualized care plans.
- Educate patients and caregivers on mobility, home safety, and healthy lifestyle habits.
- Keep clinical documentation accurate and timely, helping ensure seamless coordination of care.
- Travel to patient homes within your designated area.
What We’re Looking For:For Physical Therapists (PTs):
- Bachelor’s degree in Physical Therapy
- Active Texas State License
- Valid Texas Driver’s License
- Current CPR certification
For Physical Therapist Assistants (PTAs):
- Active Texas State License
- Valid Texas Driver’s License
- Current CPR certification
- Strong communication and documentation skills
