What You’ll Do:- Evaluate patients and develop individualized therapy plans
- Provide in-home physical therapy treatments
- Improve mobility, strength, balance, and functional independence
- Educate patients and caregivers on exercises and safety techniques
- Monitor progress and adjust treatment plans as needed
- Complete accurate and timely documentation
- Active Texas Physical Therapist (PT) license (required)
- Home health experience preferred
- Strong clinical judgment and patient care skills
- Ability to work independently in a home setting
- Excellent communication and documentation skills
- Reliable transportation & valid driver’s license
- Kinnser/WellSky EMR experience (preferred)
- Up to $75 per visit
