Role Description
This is a part-time, remote role for Home Health professionals, including RNs, LVNs, PTs, OTs, STs, Aides, and full time liaisons and full time office staff. Responsibilities include providing in-home health care services, participating in patient assessments, delivering skilled nursing or therapy care, and maintaining accurate patient records. Collaboration with interdisciplinary teams, administering treatments, managing patient progress, and offering compassionate support to patients and families are integral parts of this role.
