East Valley Arizona; Chandler, Mesa, Gilbert, Tempe, Queen Creek, Apache Junction, and Scottsdale. Fountain Hills, Deer Valley, North and South Phoenix, Wittman.

We are currently seeking experienced and compassionate healthcare professionals for per-visit positions:

Position Overview

As a member of our Home Health team, you will provide skilled care and support to patients in their homes, helping them achieve optimal health outcomes while maintaining their independence and quality of life.

Qualifications

• Current and active Arizona license or certification appropriate to the position
• Minimum of one (1) year of clinical experience preferred
• Home Health experience is a plus but not required
• Strong assessment, communication, and documentation skills
• Ability to work independently and manage time effectively
• Current CPR certification (if applicable)
• Reliable transportation, valid driver's license, and current auto insurance
• Ability to travel within service areas throughout the Valley

Responsibilities

• Provide skilled nursing, therapy, social work, or aide services according to the patient's plan of care
• Complete timely and accurate documentation
• Collaborate with physicians, case managers, and interdisciplinary team members
• Educate patients and caregivers regarding treatment plans and disease management
• Maintain compliance with agency policies, state regulations, and Medicare guidelines
