We are seeking a passionate and outgoing Community Liaison to represent our Home Health agency in the community. In this role, you’ll promote our services, develop referral relationships, and help connect patients and families with the care they need.
Responsibilities:
- Build and maintain relationships with physicians, hospitals, senior living communities, and other referral sources
- Promote home health services through regular outreach and inperson visits
- Attend community events, health fairs, and networking functions
- Serve as a knowledgeable and professional representative of our agency
- Track outreach efforts and assist in meeting census growth goals
Requirements:
- Experience in home health, hospice, healthcare marketing, or community outreach (preferred)
- Strong communication and relationshipbuilding skills
- Knowledge of local healthcare providers and referral networks
- Reliable transportation and valid driver’s license (travel required)
