Travel MDS Nurse (RN)
Are you an experienced MDS Coordinator looking for flexibility, variety, and a chance to make a real impact?
Evergreen Healthcare Group is hiring a Travel MDS Nurse to support multiple skilled nursing facilities across multiple states. Hybrid role with some flexibility on remote work and travel between three states (NV, OR, WA, MT, SD, WY). Covering in the centers as the MDS Coordinator as needed. Compact RN licensure is a requirement; NV RN licensure is a plus!.
What You’ll Do:
Support clinical teams with timely and accurate MDS assessments
Coordinate ARDs and IDT participation across multiple facilities
Conduct on-site visits for training, audits, and assessment support
Utilize EMR systems to complete MDS documentation and ensure regulatory compliance
Promote quality outcomes through collaboration with facility leadership
Step in to support urgent MDS coverage needs at facilities experiencing staffing gaps, including temporarily filling in during transitions until a permanent replacement is hired.
Requirements:
Compact and single state RN license to support NV, OR, WA, WY, SD, MT (current and unencumbered)
4 years as an MDS Coordinator in a skilled nursing facility
Strong understanding of the RAI process and care planning
Comfortable with travel across multiple states (West and Mid-West)
Proficient in EMR systems (PointClickCare preferred)
Preferred:
RAC-CT Certification
Medicare utilization review experience
Strong documentation and analytical skills
