Behavioral Health Provider
Valley-Wide is an approved site for the Colorado Health Service Corps and National Health Service Corps loan repayment programs, offering eligible providers up to $90,000 for delivering care in our rural communities.
About Us
We are a non-profit, Federally Qualified Health Center (FQHC) dedicated to serving diverse populations across a multi-county area in southern Colorado. With 34 strategically located service sites, we provide trusted primary and preventive care throughout the region.
Position Overview
In this role, you will be part of an integrated, primary healthcare network using a Team-Based Care (TBC) approach. Your focus will be on holistic, preventive, therapeutic, and educational services for patients of all ages while promoting patient and family-centered care.
Job Responsibilities
- Clinical Care: Perform assessments, identify problems, justify medical necessity (using DSM-5 criteria), and deliver evidence-based treatments, crisis intervention, and individual/family/group therapy.
- Collaboration: Work within the Primary Care Behavioral Health (PCBH) model to develop and implement treatment plans through consultation, brief assessments, and psychoeducation.
- Documentation: Maintain accurate and up-to-date patient records in our EMR system (NextGen), including intake documentation and treatment plans.
- Compliance: Uphold HIPAA regulations and patient confidentiality at all times.
- Additional Requirements:
- Bilingual skills in English/Spanish are desirable but not required.
- Must successfully complete drug testing and background screening.

Qualifications
- Educational Background:
- A master’s degree in psychology, counseling, or clinical social is required
- Licensure:
- Candidates may hold a current license with the Colorado State Board as any of the following:
- Licensed Clinical Social Worker (LCSW)
- Licensed Professional Counselor (LPC)
- Licensed Marriage and Family Therapist (LMFT)
