Location: Upper Valley area
We are seeking an experienced LPN or RN to serve as Health Services Director at a senior living community in the Upper Valley. This hands-on leadership role oversees resident care, leads the nursing and care team, and supports high-quality services in a welcoming, resident-centered environment.
The ideal candidate is an organized, compassionate leader with at least five years of nursing experience and previous management experience. A background in senior living and memory care is preferred.
Responsibilities
- Oversee daily health services and coordinate care to meet residents’ individual needs.
- Lead, support, and supervise nursing and care staff.
- Complete and oversee resident assessments, care planning, and follow-up.
- Support compliance with applicable regulations, community policies, and quality standards.
- Coordinate with healthcare providers, home health agencies, and hospice services.
- Maintain accurate, confidential resident records.
- Communicate effectively with residents, families, staff, and outside providers.
- Manage departmental resources and support budgeting and staffing decisions.
- Provide staff education, coaching, and ongoing performance feedback.
- Identify opportunities to improve care delivery while respecting residents’ rights, dignity, and independence.
Qualifications
- Current Vermont LPN or RN license required.
- At least five years of nursing experience.
- Previous management or supervisory experience.
- Senior living and memory care experience preferred.
- Strong leadership, communication, and organizational skills.
- Experience with resident assessments, care planning, and medical documentation.
- Ability to manage resources, prioritize responsibilities, and work collaboratively.
- Familiarity with home health and hospice services preferred.
- Commitment to compassionate, resident-centered care.
Pay: From $80,000.00 per year
