Job Description
The Thoracic Nurse Navigator is a key position in the Valley Health Pulmonary and Thoracic Oncology Program. This position focuses on managing patients identified through the Lung Cancer Screening Program as well as patients with incidental lung nodule findings. The navigator will serve as an integral part of the thoracic program and will work closely with providers to expand and improve the program. Collaborating team members caring for this patient population include Radiology, Thoracic Surgery, Pulmonology, Oncology, and Primary Care Providers.
The Nurse Navigator will assure all aspects of care remain patient centered. Priorities of this navigator role include commitment to communication and education to patients, families, and the community on services available. The navigator will serve as a patient advocate and will assist patients in navigating through the thoracic program.
Duties Include, But Are Not Limited To
Enhancing access to lung cancer screenings through the Valley Health Lung Cancer Screening Program
Screening patients for eligibility for LDCT screenings including shared decision making
Managing the Lung Cancer Screening program and data base and the Incidental Lung Nodule program, ensuring compliance with screening recommendations and facilitating specialty referrals as indicated
Coordinating Thoracic Tumor Conference
Serving as a liaison between the patient and provider to communicate abnormal findings and ensure follow up as indicated
Assure patients receive timely testing and ordered interventions
Monitoring patient’s progression through the program, including compliance of care, treatment, and long-term surveillance
Education
Associates degree in Nursing (RN Diploma, ASN or AND) required
Bachelor's degree in Nursing (BSN) preferred
Experience
Nurse navigator experience preferred
Certification & Licensure
Registered Nurse with current Virginia license required
Qualifications
High level of critical thinking skills, patient assessment, creative problem solving, proficient organization, and planning
Possession of excellent written/verbal communication and interpersonal skills with all levels of internal and external customers
Demonstrated ability of working effectively as a member of an interdisciplinary team, displaying good clinical judgment and decision-making skills
Self-directed with the proven ability to work independently essential
Proficient in computer use, the Internet, and health information technology
Ability to work with diverse care providers
Ability to flex time

- years of Healthcare/Medical Registered Nurse Oncology, Oncology Navigation experience preferred
