What are the responsibilities and job description for the Patient Navigator position at Valley Forge Medical?
We are currently seeking a highly organized and compassionate individual to join our team as a Patient Care Navigator. The Patient Care Navigator will be responsible for ensuring that patients admitted to our addiction and recovery hospital are able to attend all their appointments with providers throughout the day.
Key Responsibilities:

Coordinate patient movement within the hospital to ensure that patients can attend all their scheduled appointments with providers.
- Schedule daily appointments for all patients.
- Communicate with patients and their families to provide information about the patient's schedule, including appointment times and locations.
- Communicate with providers to ensure that patient appointments are scheduled in a timely manner and that any necessary changes are made in a timely manner.
- Escort patients to appointments as needed, helping with mobility and transportation as needed.
- Ensure that patient privacy and confidentiality is always maintained.
- Keep accurate and detailed records of patient appointments.
- Attend meetings and training sessions as needed.
- Other duties as assigned
Qualifications:

High school diploma or equivalent
- Minimum of 2 years of experience in healthcare or related field
- Strong organizational skills and attention to detail
- Excellent communication and interpersonal skills
- Ability to work well in a team environment
- Strong problem-solving and decision-making skills
- Ability to prioritize and manage multiple tasks and projects simultaneously
- Understanding of addiction and recovery a plus
