JOB TITLE: IDD Engagement Support/Hospital Liaison Case Manager
DIVISION: IDD
REPORTS TO: Director IDD Services
FLSA STATUS: NON-EXEMPT
WORK HOURS: Forty Hours per Week
NATURE OF WORK:
The Hospital Liaison acts as a link between Sharpe Hospital IDD patients and Agencies throughout the state to locate potential placement.
The Hospital Liaison will spend time meeting potential consumers in the hospital setting to determine client needs to transition back into the community.
The Liaison will develop a network of community resources with key stakeholders in order to share and gain knowledge of available resources, identify gaps, and promote development of resources.
Ensure, through supportive and preventive services, that at-risk individuals can live securely in the community of their choice with the necessary supports needed to be successful.
Ensure, through intensive follow up and support, the individuals discharged from inpatient psychiatric hospital are surrounded by community resources that will strengthen their recovery and resilience in the community.
WORK ENVIRONMENT: (In-Doors 75% Out-doors25%)
WORK PACE: Self-paced, requiring the ability to make decisions quickly and accurately in an oftentimes fast-paced, demanding work environment.
ESSENTIAL DUTIES of the Engagement Specialist/Hospital Liaison:
- Will work at Sharpe Hospital at a minimum of 3 days per week.
- Attendance/participation and treatment team/discharge planning meetings at assigned psychiatric hospitals for any consumer that is potentially being transitioned back into the community.
- Make at least 3 referrals per week to Agencies that are accepting referrals for client placement in the community to include sending referral packets of information to the Agency.
- Provide information regarding availability of potential placement for IDD patients of Sharpe Hospital.
- Completion of applications for benefits for clients being discharged from the hospital.
- Assistance securing housing, benefits, necessities to clients being discharged from the hospital and moving into their own residence.
- Attends all mandated trainings related to profession and enhances knowledge and/or skills which include Relias Learning Trainings.
- Adheres to Valley’s Standards of Conduct and Compliance and follow ethical guidelines of their profession.
- Promotes appropriate community interactions by handling public interactions in a professional and competent manner. Returns telephone calls and requests for information in a timely manner, promotes a positive image of the agency, and maintains a good rapport with agency contacts.

MINIMUM QUALIFICATIONS:
- Ability to perform Essential Duties as outlined below.
- BA/BS in Human Service Field.
- Six months to one-year experience with IDD population.
- Ability to comply with all of Valley’s Policies and procedures.
- Ability to comply with Client’s Rights.
- Ability to comply with Valley’s and Division departmental safety procedures.
- Ability to read, write and speak the English language.
- Ability to legally transport clients.
- Current CPR/First Aid Certificate or ability to be trained.
- Excellent Written and Oral Communication skills.
PREFERRED QUALIFICATIONS:
- LSW or eligible for TLSW
- Greater than 1 year experience with MI population.
