Purpose of Position
The Quality Assurance Associate is responsible for assisting in assessing and analyzing, the quality, effectiveness, and compliance of each individual’s active treatment program, residential setting, day program, and non-residential setting in accordance with the state regulations. The Quality Assurance Associate is also responsible for coordinating the medication administration training, core indicator survey, and quality of life survey for VCS. This person works with the direction and supervision of the Director of Quality Assurance in accordance with the functional precedents, practices, and policies of Valley Community Services.
Qualifications
Education/Experience: High School equivalent and 4 years’ experience or Associates degree in quality, business, human services or related field and 2 years’ experience with intellectual disabilities programming, or a bachelor’s degree and 1 year experience with intellectual disabilities programming. Associate's degree preferred.
Specialized Skills:
1. Working knowledge of theories, principles and practices applicable to the services provided to clients of Valley Community Services.
2. Ability to prepare clear, concise reports and effectively communicate orally and in writing.
3. Sensitivity in relating to persons of varying backgrounds, demonstrated ability to work with diverse groups of people possessing various strengths, aptitudes, and abilities.
4. Ability to perform job responsibilities with a high degree of initiative and independent judgment.
Principal Accountabilities
Essential Functions:
A. Quality Assurance
Through chart reviews, the Quality Assurance & Training Associate will aid in the assessment of active treatment program, residential program, day program and non-residential programs to ensure compliance with the state regulations.
Responsibilities are listed below:
1. Completes chart reviews on all people living in 6400 homes, 6500 homes,
and Non-Residential programs in accordance with 6100 regulations.
2. Compiles the results of the analysis to the Director of Quality Assurance &
Training.
3. Distributes, collates, and analyses of both the agency’s annual core indicator and quality of life survey.
4. Assists in the follow up and/or corrective actions needed addressed from the analysis of the core indicator and quality of life survey.
5. Assists in the monitoring of the Quality Management Plan with quarterly and annual reports.
B. Program Licensing
1. Ensures that all 6400 homes, 6500 homes, and Non-Residential programs are in compliance with governmental regulations.
2. Assists in the preparation for annual licensing. Including: scheduling, organizing, and participating in self-inspections.
3. Attends all licensing entrances and exits within assigned region.
4. Follows POCs and CAPs to ensure that the corrections have or are occurring as specified.
5. Assists in the coordination of all CLA, LifeSharing, and Non-Residential licensing documents.
6. Supports the Director of Quality Assurance in the completion of the annual QA&I process.
7. Assists in the preparation and monitoring of IM4Q’s, SIS’, and high-profile Individual Plan Meetings. Including: scheduling, organizing, and participating in external governing bodies’ meetings.
8. Assists with completion of request for CLS annual licensing and Department of Health documentation during site licensing for all people living in the ICF/ID homes.
C. Program Development
1. Coordinates needed items for all new admissions, including coordination of admission packet information with Supports Coordinators and family members of persons who will be served.
2. Create and organize individual’s charts and CLA home reference manuals.
3. Assists in the distribution of the annual ICF IPP schedule and CLA quarterly schedules.
4. Assists in ODP requirements opening of new CLA homes.
D. Medication Administration
1. Oversees and maintains all administrator’s files, within their region, per ODP guidelines for completeness and accuracy.
2. Creates and monitors online class registration and completion of medication administration students.
3. Teaches medication administration course and ensures totality of documentation for new medication administrators.
4. Trains and supervises Practicum Observers.
5. Assists in medication administration error re-trainings when necessary.
E. Training
1. Assists Director of Quality Assurance with creation, modification, and updating of training manuals.
2. Attends Operation Director’s Program Specialist/Program Manager meetings to disseminate new information, clarify current topics of concern, and answer questions regarding regulations. Information collected from meetings are reviewed with the Director of Quality Assurance to ascertain systematic concerns needing remediation.
3. Participates in house meetings, when necessary, to educate front line staff concerning specific matter in regard to the home and/or persons served.
4. Disseminate correspondence from governing bodies to appropriate personnel and departments.
5. Assists in obtaining information necessary in creating agency policies.
6. Gathers information from appropriate ancillary departments to ensure accuracy of policies.
7. Assists the Director of Quality Assurance creating procedures and guidelines for identifiable polices.
Other Mission Related Duties as Assigned:
1. May be required to become a Certified Investigator via training sponsored by the
Department of Public Welfare.
2. May be required to become a member of the Investigatory Review Team; a body that reviews HCSIS reportable and Human Resource incidents and makes administrative recommendations to the Executive Director.
3. May be required to become a member of the Workman’s Compensation Safety
Committee; a committee formed to make health and safety recommendations to the Executive Director.
