Duties
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs.
- Develop and implement individualized care plans that align with clinical protocols and patient goals.
- Coordinate discharge planning and post-acute care arrangements to ensure smooth transitions from hospital to home or other facilities.
- Manage utilization review processes by evaluating medical necessity, reviewing diagnostic evaluations, and supporting appropriate resource allocation.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals .
Pay: $85,318.79 - $102,749.51 per year
