What are the responsibilities and job description for the Mobile Integrated Health Community Paramedic position at UVM Health - UVM Medical Center?
The UVM HealthNet Community Paramedic functions as an integral member of a multidisciplinary healthcare team, planning, providing, and evaluating direct care to patients in all states of health and wellness in both pre & post hospital settings. The Community Paramedic will collaborate and align closely with a variety of local social services, case managers, area medical facilities, private insurance companies, private practices and public housing authorities; assisting with necessary treatment (s) and care of patients inside their homes, on the streets, and in many other private & public settings. The community paramedic will cultivate essential working relationships between patients and their healthcare network while helping the patient navigate the healthcare system overall to improve their care, reduce costly readmissions to Emergency Departments and reduce misuse or overutilization of area 911 systems.
Education
Education
- High School Diploma or Equivalency (GED) required
- Nationally Registered Paramedic Certification
- Associate’s or Bachelor’s Degree preferred
- CP-C (Community Paramedic Certification) preferred after 12-to-24 months of hire
- 3 or more years of direct patient care experience in 911, critical care, or community paramedicine.
- New York Emergency Medical Services Paramedic Certification (preferred)
- Vermont Office of Emergency Medical Services Paramedic Certification (required)
- National Registry Emergency Medical Technician Paramedic (NREMT-P) (required)
