Schedule: Monday-Friday, 9:00 AM-5:30 PM
Purpose Statement/Position Summary: This position will be supporting the NATEEN Teen Parent Support Program at CHLA and will include responsibilities for providing case management services to expectant and parenting youth, home visitation, linking youth to community partners, assistance with school enrollment, health education, and other resources. Under the supervision of a Clinical Social Worker, the Social Worker Associate is responsible for providing comprehensive youth transitional case management services to at risk youth At-risk areas which may include the prevention and treatment of HIV, substance abuse or other At-risk condition. Will be responsible for assisting with follow up on referrals and data entry. This position will work closely with other Care Services team members to ensure that youth are enrolled in appropriate services. Responsible for the development of relationships with community-based agencies and providing technical assistance and training as requested. Participates in various community meetings.
Minimum Qualifications/Work Experience:
Required:
- Valid CA Driverâs License and proof of insurance or access to reliable transportation may be required for positions requiring off-site duties.
Preferred:
- 2 yearsâ experience providing case management or clinical services to At-risk adolescents.
- Experience in providing services to At-risk youth.
- Experience in working with diverse populations.
- Familiar with positive youth development frameworks.
- Knowledgeable about adolescent development and sexual health.
- Health education experience (contraception, sexual decision-making, and/or family planning).
- Interested in working with adolescent population and capable of building rapport with youth.
Education/Licensure/Certifications: Bachelorâs Degree required. American Heart Association BLS certification required.

The pay scale listed for this position is generally for candidates who meet the specified qualifications and requirements listed on this specific job description.
