Overview
Purpose Statement/Position Summary: The Care Coordination Associate assists in facilitating efficient functioning for patients within the Cancer & Blood Disease Institute (CBDI) and on-site support of providers during clinic sessions. Responsible for coordination of the patientâs visit in the clinic. Prepares, compiles, and maintains appropriate records and reports in accordance with established guidelines and procedures. Coordinates of highly complex patient visits in a high patient volume specialty with inter-professional patient care providers. Demonstrates commitment to a patient and family focused model of care.
Minimum Qualifications/Work Experience: Minimum of one-year related clerical or equivalent experience in a medical setting. Experience in clinical data entry experience in medical transcription or equivalent training. Acquired knowledge in an ambulatory pediatric environment preferred. Effective verbal and written communication skills. Broad knowledge of computer software including word processing, spreadsheets and presentation applications; keyboarding skill level at 45wpm. Preferred for positions in medical/clinical areas: basic medical and scientific terminology. Bilingual preferred. [If fluent in another language, assist in interpreting on a limited basis (phone calls to families to check demo info, brief dc planning on phone, discuss learning needs/expectations). NOT for extended teaching, not to translate DC orders/instruction in KIDS.] Language proficiency testing required according to policy MHR 23.1
Education/Licensure/Certifications: High School graduate or equivalent. Training or experience in healthcare field, familiar with health care terminology.
Schedule : Monday - Friday, 8:30am-5:00pm.
