Your Role & Responsibilities
As the RNAC / MDS Coordinator, you will:
- Conduct routine resident assessments according to established schedules and as needed for condition changes, hospital discharges, and other clinical events
- Accurately observe, assess, and communicate changes in resident condition to appropriate interdisciplinary team members
- Collaborate with Social Services, Activities, Dietary, and Nursing to ensure timely completion of assessments and CAAs with appropriate documentation
- Complete MDS sections, quarterly reviews, CAAs, and care plans in compliance with state and federal regulations
- Develop, update, and monitor individualized resident care plans based on assessment findings
- Prepare and distribute the MDS schedule to the interdisciplinary team
- Transmit MDS data to the state weekly (or as required) and maintain accurate, organized records
- Current Registered Nurse (RN) license or Licensed Practical Nurse (LPN) in the state of Iowa
- Prior experience working as a Coordinator in a skilled nursing or long-term care setting
- Strong knowledge of MDS processes, assessment timelines, CAAs, and care planning requirements
- Excellent organizational, communication, and collaboration skills
