Position Summary:
We are presently seeking dedicated professionals to join Urban Health Plan (UHP) in the capacity of Licensed Practical Nurse. We are currently recruiting a full-time nurse to join our team in the Bronx, NY working in our Center for Health Aging Department.
Minimum Qualifications
Minimum $63,960 starting pay; Maximum $72,560/annum based on 40 regular hours per week based on advanced experience.
Work Environment Type, Location, and Schedule:
Onsite, Bronx, NY, Monday to Friday 8:00am-5:00pm/9-6- with rotating Saturday shift schedule.

Minimum Qualifications
- High School Diploma required LPN Training Program.
- NYS Licensed Practical Nurse Licensure required.
- Skilled in phlebotomy and EKGs.
- All experience levels are welcome to apply, whether this is your first role as a nurse, or you are coming in as a seasoned nurse.
- Bilingual English/Spanish is required.
- Knowledge/training in electronic health records and Microsoft Office.
- Performs vitals, comprehensive screenings, and assessments (under supervision of RN).
- Provides vaccinations and blood draws.
- Assists with prepping patients as needed and providing patient education.
- Assists providers during examinations and treatments.
- Ensure proper sterilization procedures using germicides, sterilizers, or autoclave.
- Ensures departmental adherence to clinical policies and procedures.
- Maintains health records by documenting patient contact and updating patient profile.
