What are the responsibilities and job description for the Scheduler position at UpStream?
PURPOSE
The Scheduler is responsible for efficiently coordinating appointments, managing waitlists, and ensuring smooth communication between clients, staff, and departments. This role requires strong organizational skills, attention to detail, and a commitment to maintaining a professional and responsive work environment.
KEY RESPONSIBILITIES
- Maintain all office functions.
- Work in tandem with benefits coordinator and all Providers for scheduling needs in the electronic health record (EHR) system.
- Support Medical Providers by taking vital signs.
- Chart management - must be able to navigate an electronic health record system to support record requests for clients and community agencies and court systems.
- Manage Telehealth session logistics for intake, screenings, and assessments with staff available via Telehealth.
- Make appointments, track and call and send out via the EHR appointment reminders.
- Coordinate, manage and send electronic and written health records to external agencies with previous verification of signed releases of information per Upstream policies and procedures. This includes progress, counseling, DAP notes and clinical documentation.
- Works in collaboration with the assigned team, regional supervisors, and leadership while following the appropriate processes.
- Clean and sanitize facility as needed.
- Collaborate with other agencies and staff.
- Have AMAZING customer service skills and de-escalate clients and people walking in if needed.
- Answering phones and transferring calls, uploading documents to charts.
- Maintains professional and ethical behaviors with all clients and staff.

QUALIFICATIONS
- Ability to work independently and as part of a team in a fast-paced environment.

- Previous experience in scheduling, administrative support, or customer service preferred.
