Position Summary

UVS is seeking a dedicated, organized, and compassionate Specialized Service Coordinator to provide and coordinate support services for individuals with developmental disabilities and co-occurring mental health needs who require specialized care.

As a Specialized Service Coordinator, you will serve as the primary liaison between individuals, families, Shared Living Providers, community partners, healthcare professionals, and state systems. You will help ensure that services are person-centered, responsive, compliant, and aligned with each individual's goals and needs.

Thorough initial and ongoing training is provided.

Key Responsibilities

Person-Centered Planning & Support

  • Facilitate the development, implementation, and ongoing review of individualized service plans
  • Collaborate with individuals, families, providers, and support teams to identify goals and services
  • Advocate for participant rights, independence, and community inclusion
  • Help individuals maintain housing, income sources, and community living skills

Service Coordination & Case Management

  • Coordinate and monitor all services for an assigned caseload
  • Serve as the primary resource and liaison to Shared Living Providers and families
  • Conduct regular home visits and maintain ongoing communication with providers and participants
  • Coordinate healthcare, community engagement, and other needed services
  • Consult with treatment teams and community partners regarding assessment, rehabilitation, and support needs

Health, Wellness & Specialized Support

  • Ensure compliance with Agency and Division of Developmental Services Health and Wellness Guidelines
  • Monitor participant well-being, medications, and treatment plans
  • Coordinate supportive counseling, medication education, mental health education, and early crisis intervention when needed
  • Monitor service effectiveness and recommend modifications as appropriate

Documentation & Compliance

  • Maintain complete, accurate, and timely case records
  • Submit required reports and documentation according to agency and state standards
  • Track progress toward goals and ensure regulatory compliance
  • Stay informed of agency policies, state regulations, and legislative changes affecting individuals with developmental disabilities

Crisis Response & Quality Assurance

  • Respond to urgent situations with professionalism and sound judgment
  • Support families and providers through challenges and crisis situations
  • Reassess service plans and implement approved changes as needed
  • Ensure services meet quality standards and regulatory requirements

Leadership & Collaboration

  • Participate in agency, community, and statewide committees as appropriate
  • Provide oversight and support to assigned residential programs and staff
  • Coordinate respite services for families and Shared Living Providers
  • Work collaboratively with interdisciplinary teams to ensure high-quality service delivery

Additional Duties

  • Transport individuals to appointments or service-related activities as needed
  • Provide occasional staff coverage when required
  • Perform other duties as assigned

Qualifications

Preferred Education & Experience

  • Bachelor's degree in Human Services or a related field, or an equivalent combination of education and relevant experience
  • Experience supporting individuals with developmental disabilities required; experience with mental health services preferred
  • Strong knowledge of person-centered planning and community-based supports

Required Skills

  • Excellent organizational and time-management skills
  • Strong written and verbal communication abilities
  • Ability to prioritize tasks and meet deadlines
  • Sound decision-making and problem-solving skills
  • Proficiency with documentation and computer systems
  • Ability to work both independently and collaboratively

Additional Requirements

  • Valid driver's license
  • Reliable, insured vehicle
  • Ability to pass required background checks

Schedule & Location

  • Monday–Friday, 8:00 AM – 4:00 PM
  • Occasional evening or weekend availability as needed
  • Some on call work is needed, comes with additional compensation
  • Hybrid role combining fieldwork, and office-based work
  • Office location: Waterbury, Vermont
