Our program brings the region’s experts in geriatric medicine and care together to work as a team in specialized Day Centers, to help older adults enjoy the highest quality of life possible. Our professionals are committed to keeping older adults independent, and in their homes. Our wide range of services are designed to meet the varied needs of seniors, and include medical care, social services, meals, activities, transportation and much more.
Schedule: Full-Time M-F, 7:30am - 4:00pm
Location: Community LIFE service areas in PA (mileage is reimbursed at federal rate)
Summary of Job Duties:
The position will provide social services to seriously and terminally ill patients and their families in a PACE setting. The Social Worker will be part of the Supportive Care team and act as a consult member serving the Community LIFE’s primary teams. The Social Worker will participate in the development, implementation and coordination of psycho-social services in order to allow frail elderly participants to remain at home through end of life.
Required Education: Master’s Degree in Social Work (MSW) from an accredited school of social work.
Required Experience: Minimum of 1 year experience with the frail or elder population, Preferred minimum of 3 years’ experience working in a palliative - hospice program.
Required Skills:
- Good verbal and written communication skills.
- Ability to work with people from various socio‑cultural backgrounds in an effective manner.
- Ability to accept and utilize supervision.
- Ability to work independently and be self directed.
- Good problem solving skills.
- Must have a compassionate and empathetic attitude for the frail older adults.
Required Certifications/Licensing: Valid PA Driver’s license & reliable vehicle
Compensation details: 26-34 Hourly Wage
