Somerset Health Services Inc is hiring a Full-Time Physician Assistant or Certified Registered Nurse Practitioner to help support Davidsville Family Practice.

Hours: Monday-Friday 8:00 am - 4:30 pm
Location: 143 Woodstown Hwy, Hollsopple, PA 15935
Position Specifics: Provider would hold an independent patient panel in which they would be responsible for new, established and acute visits, annual wellness visits; order appropriate testing, review results and management patient conditions appropriately; manage provider in-basket messages from patients, staff and other providers;  Appropriately refer patients to specialists when warranted; collaborate with supervising or collaborating physician; complete CME, licensure and certification requirements; complete accurate and timely documentation of patient encounters, messages and review of testing results.
Department Details: Davidsville Family Practice is one of four primary care practice sites under Somerset Health Services with the other three sites being in Berlin and the Somerset area.  The Davidsville facility is located at the Somerset/Cambria county border.  The facility itself is a newly build brick building located just off of Rt 219.  The practice is very busy and has expanded access year over year since opening.  The practice has two primary care physicians and one APP. The APP reports directly to the Sr. Director of Operations for Somerset Health Services and works very closely with the two physicians in the practice.  There is also an onsite Practice Manager for this location.

Staff/Senior Level:
The Staff or Senior Advanced Practice Provider (APP) is a Physician Assistant or Certified Registered Nurse Practitioner and a member of the care delivery team. The APP is responsible to set the standards for the level and quality of care. The APP has responsibility, authority, and accountability for the provision of care through supervision by and collaboration with a physician. The APP manages and provides patient care activities for a group of patients and their families through the application of independent judgment, communication, and collaboration with all team members. The role of the APP encompasses the beginnings of leadership, partnership, collaboration, and supervision. The APP establishes and maintains collaborative relationships with physicians, other health care providers, patients, and their families, to achieve desired patient outcomes throughout the continuum of care. The APP begins to demonstrate a commitment to the community and to their healthcare profession.

The successful candidate will be placed according to years and months of experience for employment purposes.

• Actively engages in clinical development of all members of the healthcare team. Requests opportunities to learn advanced professional practice techniques from senior healthcare team members. Demonstrates enthusiasm for continuous learning and identifies and creates a plan for the continuation of learning and development. Identifies patient and family needs for education and provides basic education to support the episode of care.
