UPMC Home Healthcare and Family Hospice is seeking a compassionate and dependable Home Health Aide to provide exceptional, one-on-one care to home health and hospice patients throughout Erie County. This field-based role offers the opportunity to make a meaningful impact by supporting patients in the comfort and dignity of their own homes. This position is a full-time, Monday-Friday position from 8:00AM-4:30PM with rotating weekend shifts.
Responsibilities include:
Providing safe, direct personal care under the supervision of licensed professionals
Monitoring patients’ conditions and promptly reporting changes to the interdisciplinary care team
Supporting patient comfort, well-being, and overall satisfaction
Completing accurate and timely documentation in accordance with agency and regulatory standards
Collaborating effectively with team members to ensure coordinated, high-quality care
Participating in required meetings, training, and annual competency evaluations
Adhering to organizational policies, standards of conduct, and compliance requirements
