Position: PRN Licensed Practical Nurse (LPN/LVN)
Location: Polk County
Schedule: Weekends (as needed)
What You’ll Do:
- Provide skilled nursing care in the home setting under the direction of an RN
- Follow physician orders and individualized plan of care
- Monitor patient conditions and report changes to the RN and physician
- Administer treatments and perform nursing procedures per agency standards
- Educate patients and caregivers on medications, disease processes, and home care
- Document visits in compliance with Medicare guidelines
- Ensure adherence to HIPAA, infection control, and regulatory requirements
Qualifications/Educational Requirements:
- Licensed Practical Nurse (LPN) from a state-approved, accredited program.
- Oasis Home Health Care 1 year of experience.
- Minimum 2 years of healthcare experience
- Strong communication and organizational skills
- Ability to work independently and manage time effectively
- Understanding of Medicare/Medicaid regulations (preferred)
- Reliable transportation for travel to patient homes
- Must have AHCA II Background Check: https://info.flclearinghouse.com
- Must have a valid Driver’s License and Auto Insurance
- Alzheimer and Dementia 1 year of verifiable experience
- Current CPR
Field Responsibilities/essential functions: : The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.
1. Follow-up assessment and documentation of observed patient's condition and interventions provided at each visit
2. Complete patient evaluation tasks, including medication administration and vital signs
3. Administer Physician-prescribed medications
4. Wound care and maintenance of skin integrity
5. Reinforce patient and family education on appropriate home health care procedures and strategies
6. Coordinate with RNs, physicians, physical therapists, occupational therapists and other individuals in the patient's care plan
7. · Ensure patient’s comfort while supporting independence
